ASSUMPTION OF RISK, WAIVER OF LIABILITY, AND MEDICAL AUTHORIZATION
In consideration for allowing my child(ren) to use these facilities, I, on my behalf of my child(ren) and as legal parent/guardian, I recognize what potentially severe injuries, including permanent paralysis or death can occur in sports including but not limited to fencing, special events & activities including parent participant activities and any and all other programs offered by Fargo-Moorhead Fencing Club. Being fully aware of these dangers, I voluntarily consent to the aforementioned person(s) participating in any and all Fargo-Moorhead Fencing Club programs and activities and I ACCEPT ALL RISKS associated with that participation.
In the event of an emergency, I would like my above-mentioned child(ren) or myself to be taken to a hospital for medical treatment and I hold Fargo-Moorhead Fencing Club and its representatives harmless in their execution of this action. Additionally, I hereby agree to individually provide for all possible future medical expenses which may be incurred by my child as a result of any injury sustained while participating at or for Fargo-Moorhead Fencing Club.
I have read and understood this ASSUMPTION OF RISK, WAIVER OF LIABILITY & MEDICAL AUTHORIZATION.
CONSENT TO MEDIA:
By your attendance in class or events at Fargo-Moorhead Fencing Club, you are granting your permission for you and your child to be filmed, videotaped, audio taped, or photographed by any means and are granting full use of your likeness, voice, and words without compensation.
Club Policies:
In order to get the equipment discount by the club the student must have their own jacket, mask, body cord/s and electric weapon/s. If a student practices two different modalities (e. g. foil and epee), in order to apply for the discount the student must have at least one weapon of each modality (e.g. one epee AND one foil)
Club weapons are ONLY available to those members that have paid regular (not-discounted) club fees, in other words, those that have paid discounted price CANNOT use club weapons, for those fencers that pay discounted price because they have their own equipment, if a weapon breaks while practicing, assuming the fencer/athlete that just broke the weapon doesn't have a spare one, he/she can use the club weapon that night ONLY, he or she is suppose to buy one extra blade (either from the club or any other store) and repair his/her own weapon before the next class, on the next class he/she WONT be able to use club weapons.
If you are caught leaving club equipment on the floor, fridge, table, wherever not on a hanger or the shelves that we have, you will be dismissed from the club and not welcome back,
if you are the last one in the building it is your duty as an FMFC member to make sure that everything is in order and to report to the coach in charge if something was left out of place, after that you must organize it as it should have been and lacking to do so will incur in a dismissal of 1 month from the club and a probation period of three months after that.
